The Flemish Patients’ Platform respects your rights when processing your personal data. In this privacy statement you will find information about how we process your personal data within Goal Finder. This privacy statement outlines our general policy on data processing and protection.

In this privacy statement we use the personal pronoun we to refer to the Flemish Patients’ Platform.

Which personal data do we process and for what purpose?

We process your personal data when you register for the Goal Finder web application, to enable the use of the Goal Finder web application and to map out your life goals. We do this based on your permission.

Below you will find all the purposes – reasons – why we process personal data within the Goal Finder application:

  1. To create an account on the application.
  2. To register users and log in to the Goal Finder application.
  3. To identify users.
  4. To ensure that healthcare providers can confidently find the right patient in the Goal Finder application. In order to grant access to healthcare providers and other third parties if the user gives permission for this.
  5. Consulting data from the patient’s Goal Finder application by others who were explicitly given access by the patient;
  6. To contact users if necessary.

The Flemish Patients’ Platform processes the following personal data for this purpose:

  • identification data
  • contact details
  • browsing behaviour
  • professional data
  • Goal Finder: Content entered by the user
  • Media: own images uploaded by the user

How long do we keep your personal data?

We do not store your personal data longer than necessary. More specifically, we store your personal data as long as your account on Goal Finder is active. Once your profile has been deleted, we no longer store any of your personal data.

Data deletion can also be activated if your consent is considered invalid after 1 year of inactivity. You will then receive a notification by email to reconfirm your initial consent. If confirmation is not received, your data will be deleted 1 month later.

Recipients of your personal data?

  1. You choose whether to share your completed Goal Finder with third parties or with your care provider(s). This is then your own responsibility.
  2. The organisation uses services provided by Zenjoy (‘Data processor’). Zenjoy processes personal data exclusively on behalf of the Flemish Patients’ Platform. The Flemish Patients’ Platform has entered into a Data Processing Agreement with them in which the Data Processor is obliged to comply with all obligations under Belgian privacy legislation, the European General Data Protection Regulation (GDPR) and this privacy policy.

Processing outside the EEA

In principle, personal data is not processed outside the European Economic Area (EEA).

What are your rights?

When your personal data is processed by us, you have a number of rights that you can exercise.

You can request to view, receive and transfer, change or delete your personal data, and to limit or object to the processing, via isabelle.roelandts@vlaamspatientplatform.be. We will always process your request immediately and inform you of the action taken, at least within one month of receipt of the request. If the Flemish Patients’ Platform does not grant your request, for example because of other legal obligations it must comply with, it will inform you of the reasons for this.

You have the right to withdraw your consent at any time. You can contact isabelle.roelandts@vlaamspatientplatform.be for this. If we are no longer allowed or able to process your data, it is no longer possible to continue using the digital Goal Finder.

If you have any complaints about the way the Flemish Patients’ Platform processes your personal data or handles your requests, or if you have other questions or comments, you can contact isabelle.roelandts@vlaamspatientplatform.be. If this does not lead to a solution, you can of course always exercise your right to file a complaint with the Data Protection Authority (https://www.gegevensbeschermingsautoriteit.be/burger).
